Evaluation – local church (download) – Please give this form to your church chairperson, or the person who supervises you in your role. It should be emailed to your conference office.
Verification that mentor/colleague has read final paper (download) – Please download this document, save it, complete it, and email it to your conference office.
CovConnect (link) – Update your profile on CovConnect and submit it to your superintendent for approval.
